Do I need an appointment?
Yes. You may schedule an appointment by calling 718-924-2254 during health center hours.
What information should I bring with me to my appointment?
Please bring:
- A valid photo ID.
- Your current insurance card, if you have one.
- A list of current medications, including dosages, and a list of other doctors you may be seeing.
- A list of questions or concerns to discuss with your care provider.
When do I pay for the doctor's visit?
Co-pays are expected at the time of service. We accept major credit cards, cash and personal checks. We will bill your insurance or Medicaid/Medicare for the remainder of your bill. If there are additional charges, you will receive a bill from us.

What if I don't have insurance and am not eligible for Medicaid or Medicare?
We have a sliding fee scale based on family size and income in accordance with federal poverty guidelines. CHCR will never turn away a patient based on ability to pay.
Does CHCR report my immigration status to authorities?
No. CHCR does not collect or report information about immigration status to authorities.
Does CHCR have a low-cost prescription drug plan?
CHCR partners with a local community pharmacy to implement the federal 340B Prescription Drug Pricing Program, which provides reduced pricing on prescriptions for low-income patients.
How do I renew my prescriptions?
If you have refills remaining on your prescription, you may request a refill directly from your pharmacy. If you have no remaining refills, contact your doctor.
What do I do if I need an appointment on an emergency basis?
If you are experiencing a medical emergency, dial 9-1-1. We try to accommodate patients who are ill or injured with same-day appointments when possible. During business hours, call 718-924-2254.
How does the electronic records system work?
Your medical information is secure within our online system. Electronic records improve our efficiency and improve patient care. You may obtain a secure ID and password to access your records through a secure portal on this website.
