What to Bring to Your Appointment
Review this checklist for what to bring to your visit.
- List of allergies and medicines you take
- Medical and immunization records
- Emergency Department or hospital discharge records
- Valid ID: Driver’s license, or other photo ID, including US passport, foreign passport, foreign picture ID, or NY State ID
- Proof of address: valid driver’s license, posted mail with your name, utility bill, lease agreement, or insurance card
- Insurance cards (Private, Medicare, and Managed Care) and co-pay information
No Insurance
No Problem
At CHCR, we treat every patient regardless of their insurance status.
Bring proof of income, for example:
- Weekly Pay, bring four consecutive paystubs
- Biweekly pay, bring two consecutive paystubs
- Medicare recipients, bring Social Security Benefit letter
- Self-employed, bring a Self-Declaration Form

Our care coordinators team up with our insurance navigators to make sure you get the medical attention you need. They help identify how and where to get insurance for those patients who don’t have any.
